Important Messages from Mrs. Nugent

  • Hearing, Vision and Scoliosis screenings are performed on all 7th grade students whose pediatrician did not provide information. Scoliosis screenings are performed on all 6th and 8th grade students whose physician has not provided documentation of screening.

  • If your child is going to be sick, please call the Attendance Office @ 631-620-7360. The Nurses Office does not take attendance.

  • If your child is taking Medication throughout the school day, (including over the counter medication), a Medical Authorization Form MUST be completed by your physician and a parent/guardian. The form can be printed from the links or obtained in the Nurses Office. All medications, except for inhalers or over the counter eye drops, must be dropped off by a parent or guardian. If your child is going to be carrying their medication on his/her person, a Medication Release Form must be completed by the parent/guardian and your physician. A physician stamp must be present on the form.

  • Please see links below for additional forms for Diabetes, Bee Stings, Food Allergy and Asthma. These forms must be completed by the student’s physician and brought into office along with the Medication Release Form. A physician stamp must be present on the form.

  • Keep in mind that you should always keep copies of any forms that are sent to the Nurses Office.

  • Seventh Grade Physicals are Mandatory. Please provide us with a copy of your child’s physical by October 1 of the present school year. If proof of a physical is not provided, the student will then be examined by the school physician. This is mandated by the New York State Education Law. Please check in Nurses Office to make sure an up to date physical is on file.

  • Students who are entering 7th grade are required to provide proof of the Menactra immunization prior to the first day of school. Please provide proof so as not to delay your child starting 7th grade. This is in accordance with the New York State Department of Health.

  • Students who are entering 6th grade and who are 11 years of age or older MUST receive an immunization called Tdap, (Tetanus, diphtheria and pertussis), prior to the first day of school. Please provide proof so as not to delay your child starting 6th grade. This is in accordance with the New York State Department of Health.

  • If your child is 10 years old and entering 6th grade they will not be required to receive the Tdap immunization until they turn 11 years old. At that time, they MUST provide documentation of immunization or proof of an appointment within in 14 days. This is in accordance with the New York State Department of Health. Failure to provide proof of immunization will delay your child returning to school. This is in accordance with the New York State Department of Health.

  • Students entering 6th grade are also required to provide proof of TWO Varicella vaccinations prior to the first day of school. Please provide proof so as not to delay your child starting 6th grade. This is in accordance with the New York State Department of Health.

  • Make sure your child eats a healthy breakfast every day so that they may have a successful educational day.

  • If your child has an injury and is unable to participate in gym/sports, a physican note should be brought to the Nurses Office. If an elevator, crutches, buddy to carry books or early pass is needed, a physician needs to document that on the note.
