Dear North Babylon Parents/Guardians:
I am reaching out today to clarify the district’s expectations for the weekly health questionnaire.
The district has decided to use the MyWorkBadge app as a vehicle for completing the mandatory health questionnaire.
If your child attends school either every day or on a hybrid schedule, you must use the app and complete the health questionnaire for each child on Sunday, prior to the start of the school week. If your child attends the full virtual program they do not have to fill out the health survey each week.
Staff at each school will continue to take the temperature of every student in attendance each day.
The health questionnaire can be found on the website of each school. You may download a copy and send it in with your child at the start of each week. If necessary, you can call the school and ask for a hard copy of the survey to be sent home with your child.
